Health Forms

Every student attending 国产精品午夜福利清纯露脸 College must submit the required medical information forms and provide proof of comparable health insurance (with coverage in the Memphis, TN, area) before being permitted to enroll in classes. 国产精品午夜福利清纯露脸 College offers a student health insurance plan to ensure that all students have access to comprehensive health insurance in Memphis while attending the college. Students planning to participate in varsity athletics (baseball, basketball, cross country, field hockey, football, golf, lacrosse, soccer, softball, swimming/diving, tennis, track, volleyball) require additional forms. Athletic forms should be submitted by July 15.  See below for more information. 

国产精品午夜福利清纯露脸 College uses a Health Information System that shares relevant information between the Student Health Center and the Athletic Training Office to ensure quality medical care. Health Forms MUST be uploaded via Medicat Patient Portal. Submitting HEALTH forms by email, fax, or mail  is not allowed.

ALL STUDENTS

Every incoming student is required to submit several health forms to the College. Students are required to complete and submit the following forms by July 15, 2026.

  1. Medical History Form (2 pages)
  2. Proof of Health Insurance Form
  3. Physical Examination and Immunization Record Form

How to Complete Your Health Forms.

Step 1: Complete Forms via Elation Health

  • You will receive an email from Elation Health with the subject:

  • This email will include links to complete: 
    • Medical History Form (2 pages)
    • Proof of Health Insurance Form (must be valid in Tennessee)

Step 2: Upload Your Physical Examination and Immunization Form

  3. Log in using your rhodes.edu credentials (including OneLogin multi-factor authentication).
    1. Note: Only students can access this system.

ATHLETES ONLY

All student-athletes must be medically eligible in order to participate in NCAA DIII athletics.

Every varsity student-athlete must complete the following by July 15:

    Teamworks is our Athlete-Management System (AMS), which is the home to your athletic Electronic Medical Record (EMR).
    1. Use your 国产精品午夜福利清纯露脸 College email address as the Login and click Forgot Password to set up a password.
    2. Click on Favorites and complete Pre-participation Physical Evaluation (Pre-PPE) Forms.
  2. An EKG is required to participate in varsity athletics.
    1. An EKG will be provided after your arrival on campus. In the summer, you will receive more information regarding scheduling your EKG.
    2. There is a charge of $25.00 for the EKG (This will be charged to your Student Account). 
  3. Sickle Cell Trait Confirmation - Per NCAA Rules, all student-athletes must have documentation of sickle cell trait status before the student-athlete can practice, lift, or perform any team activities. Please use one of the following options:
    1. Documentation by state of birth  Please upload lab results to your Teamworks AMS Pre-PPE Portal.
    2. Sickle Cell Solubility Blood Test.
      1. Please upload lab results to Teamworks AMS Pre-PPE Portal from your personal physician.
      2. After your arrival on campus, schedule an appointment with the Health Center by e-mailing health@rhodes.edu. Before your appointment, visit the Athletic Training Room to pick up your lab order for your test.
  4. . Before classes begin, contact your primary insurance to be sure your student-athlete is covered for athletic injuries sustained outside of your normal coverage area.
    1. Student Life will email all students in July about the 国产精品午夜福利清纯露脸 College Insurance Plan. If your primary insurance does not cover your student in Memphis, you may decide to purchase this insurance plan. The 国产精品午夜福利清纯露脸 College Insurance Plan will not cover athletic injuries so a supplemental plan will be needed for athletic injuries.   5. NCAA Banned Substances - Medical Exception paperwork if applies:
    1. Exceptions may be granted for substances in the following classes of banned drugs: stimulants (ADD/ADHD meds), beta-blockers, beta-2 agonists (albuterol inhalers), diuretics, anti-estrogens, anabolic agents (steroids), and peptide hormones.
    2. If you do take a banned substance, the proper paperwork may be . Please download the forms and take them to your physician visit. Finished forms can be faxed to 901-257-0550 and will be submitted to NCAA for exemption. Student-Athletes are responsible for ensuring the paperwork is completed and returned to 国产精品午夜福利清纯露脸 Sports Medicine. 

If the proper documentation for a medical exception is not on file and the student athlete fails a drug test, the athletic training staff will not be able to appeal for a medical exception. The resulting penalty for the student-athlete may include a suspension of 365 days from the date of the positive drug test. If proper documentation is on file, we will be able to submit the medical exception to the NCAA without any penalties.
